Referral pathways
Referrals may originate from Tribal and community behavioral health partners, ACC/RBHA/T-RBHA care teams, AIHP or other authorized payer channels, hospitals, crisis providers, outpatient providers, schools, child welfare, juvenile justice partners, or guardians when clinical, legal, payer, and authorization criteria are met.
What happens after referral
The admissions team confirms receipt, identifies missing records, routes the packet for clinical review, coordinates with the guardian and payer, communicates the decision or next questions, and begins transition planning when the referral is accepted.
Cultural & Tribal partnership
DiVnity recognizes cultural identity, family connection, spirituality, language, and community belonging as protective factors. Cultural or spiritual supports are youth-guided, guardian-authorized, and coordinated with authorized Tribal or community partners.
