This notice explains how DiVnity Behavioral Health Services handles information submitted through this website. It is a website privacy notice, separate from the Notice of Privacy Practices that governs protected health information under HIPAA.

What we collect

The public website collects only the minimum information needed for the purpose of each form: general contact details for non-clinical inquiries, and structured referral information for the secure referral process. The general contact form is not intended for protected health information.

How we use it

Information submitted through this site is used to respond to inquiries, coordinate referrals, and process job applications. Referral information is routed to authorized admissions and clinical staff only.

How we protect it

The secure referral workflow is operated separately from general marketing forms and uses encryption in transit and at rest, role-based access, and audit logging. Email and text notifications about a referral never include a youth’s name, diagnosis, or other protected details.
